Register with us as a New Patient

Registering with the practice

Our patient list is now ‘open but full’ which means we review patient deductions at the end of the month and then open those spaces. Please check with our practice staff in the first instance.

In order to register with the practice, you will need to complete the following form(s)

Under 5s

  • Purple Registration Form (GMS1)
  • Summary Care Record Consent Form

5 – 16 year olds

  • Purple Registration Form (GMS1)
  • Health Check Questionnaire
  • Summary Care Record Consent Form

Over 16 year olds

  • Purple Registration Form (GMS1)
  • Health Check Questionnaire
  • Alcohol Screening Questionnaire
  • Summary Care Record Consent Form

Please may we ask that you complete ALL of the attached forms and return to us, either via post or hand them in to our reception desk, within 7 days of collecting them.   Unfortunately, if you have not returned the forms to us within the 7 days we will be obliged to offer your place to someone else.

New Patient Checks – Do I need to have a New Patient Check?

  • If you are 40 years old and over you will be asked to make an appointment to see our Health Care Assistant for a Blood Pressure check.
  • If you are 75 years old and over you will be asked to make an appointment to see our Health Care Assistant for a new patient check.

If you are on regular medication you will be asked to make an appointment for a new patient check with your GP – you will be informed of this when you drop your forms into the practice.

How we work?

In order to enhance continuity of patient care we operate a ‘personal-doctor’ registration system.
You (including children) will be allocated a named, accountable GP whom you will see for most non-urgent appointments.

If, on registration, you prefer to choose a specific doctor please inform the reception staff who will endeavour to accommodate your request depending on the availability of spaces on that doctor’s list.

When you register you will be asked to complete the following forms:

  • a registration form 
  • a brief medical questionnaire, 
  • Alcohol Screening

Once you have completed your forms we will make you an appointment with either your new GP or the practice nurse for a health check.

We have the right to refuse to register patients if they have a violent flag against their record.
